Top 10 Hospitals For Stomach Cancer Treatment In India

Stomach cancer develops when cells in the stomach start growing uncontrollably. After food is chewed and swallowed, it enters the esophagus (the tube that carries food from the mouth to the stomach), and it connects to the gastroesophageal (GE) junction, which is located just below the diaphragm. The juices in the stomach mix with the food and then move to the duodenum, which is the first part of the intestine. This area is called the stomach.

The stomach has five parts:

Cardia– It is the first part that is closest to the esophagus

Fundus– It is the part next to the cardia and the upper part of the stomach

Body (Corpus)– It is present between the lower and upper parts and is the main portion of the stomach

Antrum– It is the lower portion that is next to the small intestine. Here, the food and gastric juices mix.

Pyrolus–It is the last part of the stomach and acts as a valve (sphincter) that controls stomach contents into the small intestine.

Types of Stomach Cancer

1. Adenocarcinoma: This type of cancer develops in the innermost cells of the stomach lining.

There are two types

Intestinal adenocarcinomas – these are generally slow growing and curable. Often, this type of cancer can be treated with medical drugs and medications that are designed to target specific cancer cells. It usually occurs in older adults.

Diffuse adenocarcinoma – This type of cancer is usually not as common as intestinal adenocarcinoma. They are more aggressive and grow faster. They often metastasize quickly to other parts of the body. This usually happens at an early age.

2. Lymphomas: These cancers form in the immune system and can arise anywhere in the body where lymph tissue is present, including the abdomen. It accounts for 4% of all colon cancers. MALT (mucosa-associated lymphoid tissue) lymphoma usually develops in the stomach.

This type of cancer is seen in the lining of the stomach. Most people who are affected by MALT lymphoma of the stomach are affected by Helicobacter pylori (H. pylori) or other viral infections. It is usually a slow-growing cancer, but it can turn into a fast-growing form. If it is caused by an infection, it can be treated with antibiotics.

3. Gastrointestinal Stromal Tumors (GISTs): This is a rare type of stomach cancer that forms in special cells found in the lining of the stomach called interstitial cells of Cajal (ICC). They are present throughout the digestive tract but 60-70% are found in the stomach. These tumors can bleed and cause a blockage between the intestine and stomach resulting in pain and bloody bowel movements and vomiting.

4. Carcinoid Tumors:

They usually form in cells that produce hormones. They do not spread to other parts of the body and account for 3% of all colon cancers. They are some types.

Type I and II ECL-cell carcinoids—They rarely metastasize to other parts of the body and may not cause symptoms. They are usually discovered during endoscopy for other problems such as acid reflux.

Type III ECL-cell carcinoid – This is a more aggressive form of colon cancer. Excessive secretion of hormones by carcinoid tumors results in a condition called carcinoid syndrome. Symptoms are marked by abdominal pain, narrowing of the bronchial tubes in the lungs, diarrhea, flushing, and sometimes heart problems such as valve dysfunction.

Hereditary (familial) diffuse gastric cancer – a rare type called HDGC occurs in about 1-3% of all stomach cancers. It may be caused by a genetic mutation that is passed on to children, causing tumors to grow in multiple areas of the abdomen.

Stages of Stomach Cancer and Their Treatment in India

step 0

This condition is sometimes called high-grade dysplasia (HGD). This means that the cells of the inner lining of the stomach have been badly damaged. Abnormal cells can be removed by surgery.

This means cancer has developed in the lining, lining, or muscles of the stomach. No cancer can be found in the lymph nodes or distant parts of the body. Surgery is the main treatment option for this stage.

phase 2

The cancer may have spread to the supporting, inner muscular layer of the stomach, or it may have spread to the outer layer of the stomach. Cancer may have spread to nearby lymph nodes but not to other parts of the body. Treatment options for this stage are surgery, radiation and/or chemotherapy.

step 3

This means the cancer has spread through or into the supporting, inner or outer muscle layer. It has spread to nearby lymph nodes but has not spread to other parts of the body. Treatment options for this stage are surgery, radiation and/or chemotherapy.

step 4

Cancer may have spread to any layer of the abdominal wall or it may not have spread to nearby lymph nodes. Cancer has spread to other distant organs such as the brain, peritoneum, liver, or lungs (any T, any N, M1). Treatment options for this stage are surgery, immunotherapy, radiation and/or chemotherapy.

Treatment Options for Stomach Cancer in India

Surgery

Depending on the stage and location of the cancer, this may be done to remove the cancer and a portion of the abdomen, as well as some lymph nodes as well as other structures. Sometimes other organs also have to be removed. Palliative surgery is performed when the cancer has spread widely, doctors may still perform surgery to stop bleeding from the tumor or to prevent the tumor from blocking the stomach. Its price is 2560-5788 US dollars.

Chemotherapy

It costs USD 219-609.

Radiation therapy

It may be given before or after surgery and often with chemotherapy (radiation). This can sometimes relieve symptoms such as pain, feeding problems or bleeding. It is given for at least 5 days a week over a few weeks, but the duration of treatment depends on the reason it is given. Its price is 610-1218 US dollars.

Targeted Drug Therapy

Cancer cells have specific mutations that predispose them to proliferate. These mutations can be targeted with new and specific drugs that prevent their development. Sometimes standard chemotherapy treatment doesn’t work. They cause different types of side effects than chemotherapy. Its price is 2437-3656 US dollars.

Immunotherapy

In this, a person’s own immune system is stimulated to find and kill cancer cells more effectively. An important aspect of the immune system is not to destroy its own cells instead of other foreign or cancer cells. To achieve this, there are a number of checkpoint proteins on immune cells that act like switches that are turned on or off to initiate or end an immune response. Cancer cells can use these checkpoints to hide from immune cells. Immunotherapy drugs target these checkpoints. Its price is USD 1830-5550.

Diagnostics Tests of Stomach Cancer in India

Physical Exam and Medical History

The doctor asks about the symptoms such as flatulence, pain etc. A physical exam is done to look for possible signs of colon cancer or other health problems.

Upper Endoscopy (esophagogastroduodenoscopy or EGD)

In this, the doctor passes a flexible, light, fitted camera and thin tube down the throat. The doctor can see the inner lining of the intestine, the esophagus, and the first part of the stomach. Biopsy samples can be removed during endoscopy examination as well as during early-stage cancer.

Biopsy

If the doctor suspects an abnormality, a biopsy sample may be taken for further testing. Some stomach cancers can go deep into the stomach wall and an endoscopy may be done to take a sample. Surrounding areas such as lymph nodes or suspicious organs in the body. If cancer is found, the type of cancer is checked.

Upper Gastrointestinal (GI) series

This is an X-ray test to examine the stomach, esophagus, and first part of the small intestine. It is less commonly used than upper endoscopy to detect colon cancer and other related problems, and is less invasive than endoscopy. This involves drinking a white chocolate solution containing barium that coats the inner lining of the esophagus down to the small intestine, then several X-ray pictures are taken.

Computed Tomography (CT Scan)

It uses X-rays to make cross-sectional pictures of soft tissues. A CT scan of the abdomen tells where the cancer is. It also shows whether the cancer has spread to other parts of the body and to nearby lymph nodes. This can help in locating the extent of the cancer. CT-guided needle biopsy uses a CT scan to extract a biopsy sample from an abnormal area.

Endoscopic ultrasound (EUS)

It is usually used to see how far cancer has spread into the abdominal wall. or in nearby lymph nodes. In this, an ultrasound probe is fixed on the tip of the endoscope. Under sedation of the patient, it is passed down the throat and into the stomach. Sound waves are used to produce images of the layers of the abdominal wall.

Positron Emission Tomography (PET) Scan

This helps determine the extent of cancer. In this, a radioactive sugar is injected into the patient’s body which mainly gets deposited in cancer cells. A special camera is then used to make images of the abnormal area, although the picture obtained is not as detailed as that of an MRI or CT scan. But it can detect possible areas of cancer spread in the body.

Magnetic Resonance Imaging (MRI)

In this, strong magnets are used instead of X-rays to get a clear picture of the soft tissues in the body.

Laparoscopy

If cancer has already been detected, doctors may perform laparoscopic surgery before any other surgery, when the cancer has not spread to other areas. This involves making a small incision in the abdomen through which a flexible, thin tube with a camera is inserted while the patient is under anesthesia.

This enables the doctor to examine the lymph nodes and other nearby organs in the abdomen or to remove some tissue for biopsy. If the cancer hasn’t spread, doctors wash the abdomen with saline and peritoneal lavage to collect the fluid and check for cancer cells.

Organ Function Test

If cancer is found, the doctor may order certain lab tests if surgery is an option. Blood tests are done to check the health and function of other organs, such as the kidneys and liver. If surgery is planned, heart health and function may be checked through an echocardiogram (EKG) test.

Survival Rate of Stomach Cancer Treatment in India

The survival rate for colon cancer treatment in India is 75% after 5 years of treatment. This rate decreases with increasing stage of colon cancer.
